GETTING STARTED
Whether you are here to create a Personal History Form (New applicants) or Claim Your AZPOST Data (Current and former Arizona Peace Officers) you first need to register for an AZPOST Okta account.
IMPORTANT NOTE: The email address you use when registering should ideally never change. We recommend that you use a personal email address, as opposed to using work\school email addresses, as sometimes you can lose access to those accounts.

PERSONAL HISTORY FORM: You can use this site to create a Personal History form, aka PH. You must first register for an AZPOST Okta account. Once registered, and logged in you will be able fill in all the requested data. After you complete your original PH form, you are able to come back, edit, update and share it with multiple agencies. You never need to create another PH form.

NEW CLAIM YOUR AZPOST DATA: If you are a current or former Arizona peace officer you can now claim your AZPOST data. You must first register for an AZPOST Okta account. After successfully registering and logging in you will be able to "claim" your record.
